Hyperbaric Oxygen Therapy in Radiation Injury & Osteoradionecrosis (ORN): (Mandibular Osteoradionecrosis / Mandibular Radiation Necrosis)

Osteoradionecrosis of the mandible is a serious complication of radiation therapy in the treatment of head and neck malignancies.

It may be the result of an injury such as tooth extraction or oral surgery. This condition often occurs in the lower jawbone (mandible), a site which is exposed to radiation in cases of radiation therapy for head and neck cancers. As a result the bone tissue dies and is not able to regenerate. This process is mostly gradual and may take months or years to occur.
Radiation exposure damages important structures present in the irradiated area. The inner lining of the blood vessels supplying the area become inflamed, inadequate blood flow reaches the tissues resulting in hypoxia, loss of fibroblasts production, loss of blood vessels and the ultimate tissue death.
Hence reaching oxygen to those deprived cells is one of the primary importance of treating osteoradionecrosis.
Early seventies on, surgeons of the head and neck region came to recognize the value of hyperbaric oxygen therapy in treating damage of the maxilla and mandible occurring due to radiation treatments. Since then Hyperbaric oxygen therapy has seen some of its most dramatic successes treating osteoradionecrosis.
Hyperbaric oxygen Therapy (HBOT) can be administered as an adjunct to standard therapy to increase the amount of oxygen to reach the hypoxic tissues damaged due to radiation injuries. Hyperbaric oxygen Therapy (HBOT) stimulates the growth and proliferation of the fibroblasts, and also increases the production of collagen. Hyperbaric oxygen also increases the ability to infection-fighting white blood cells to kill harmful bacteria, accelerate healing and reduces the effects of toxic substances.

Hyperbaric Oxygen Therapy (HBOT) in reconstructive surgery involving irradiated tissue prior to surgery can help promote a well-vascularized wound and after surgery to enhance healing and the reconstructive process.
